我在 2012 學到的十件事

2012這年，我學到了很多，由於世界末日沒有來，只好 寫一下做個記錄。
理解台灣醫療系統的構成與問題

我從「公立」「醫學中心」，換到「私人」「區域醫院」，也認識了更多開業的朋友。對於三種層次的醫療照護、成本金流、政治經濟結構，有了更多的認識。
也認知到，由於健保與全民選票直接掛勾，不可能有大規模的改革，個人在裡頭安身立命還可以找到幾個位子，但整體來說，尤其對新進的年輕醫師，情況越來越艱困。
行政逐漸上手

過去的十年，我在教學、研究、醫療花了非常多的心力。今年，我在行政上開始有些心得，也逐漸有了自己的風格，我還是使用80/20法則，將科部經營效益最大化。
組織內耗少，總產出就會多。
圖、專任主治醫師群
過去一年半，我花很多心力，找出問題的解決方法。最重要的是理出秩序與可行方向，接著一一溝通，讓所有主治醫師產生共識，一起往相同方向邁進。
減少內耗，就能增加產出。這非常有威力，專任一樣是五位，平均表現卻能提高到過去的 1.5 倍。其中一位主治醫師，產能甚至達過去的 3 倍以上。
管理是魔術，但不該是騙術。
魔術，用了適當的道具以及引導手法，讓觀眾認為你完成了不可能的事情。管理，用適當的激勵制度以及觀念溝通，讓團隊完成過去以為不可能的事情。
但魔術不是什麼都能變的，沒有適當的道具、沒有適當的引導，就不可能完成。管理也是一樣，績效制度與成員熱情不到位，就不可能達到目標。
用更高的角度觀察過去的自己
圖、過去12年，congenital heart CT為重要趨勢。
今年，在亞洲心臟影像醫學會擔任先天性心臟病工作小組顧問，寫成「Cardiac CT and MRI for congenital heart disease in Asian countries」一文，讓我用更高的角度觀察過去，發現自己很幸運，在對的時候看到對的趨勢，並全力衝刺。十年間，隨著一門學問成長，成為亞洲最常被引用團隊之一。
努力是很重要，但找對方向、用對策略，一樣重要。
family life，是很昂貴的。
多少人，為了多幾百幾千萬的終身收入，或者一點虛無飄渺的權力頭銜，換得人生長久的遺憾。我們在那個當下，往往忘了，小孩的笑容、家人的幸福表情，並非理所當然。如果你身邊還有，請多珍惜，因為，這些幸福與笑容，如同一切人間事，他，並非永恆。很快小孩就會頂嘴了！
生病，是很「個人」的一件事。

生命的苦痛，從來都是個人的。那些痛、那些決定、那些意志力，都是自己的。家人再怎麼愛你，這些苦都要自己受。既然都有自己的一份，就不要給家人太多壓力了。
大概是之前 ITP 的記憶快忘了，命運就安排一個水痘給我。「這不可能無感吧？真的是有感。」
活著，不是理所當然。

我以為最不可能離婚的同學，離婚了。原來，婚姻，不是理所當然，需要去珍惜、去維護。
健康，也不是理所當然，同學中，有幾位已經過世，也有多位健康狀況不佳，勉強用現代醫療還能維持的。生命，從來都有期限，我們在期限到來前，還有遺憾嗎？
我們以為的模範生，卻是國際認證的 bumbler。
「bumble，是大智若愚、大巧若拙，沒有負面的意思。經濟學人也已經私下email給我們，說明這都是中文翻譯的錯誤。當然我們絕對不會公布這封郵件的。」
